To ensure a healthy and safe campus community, if you plan to enroll in at least one in-person class, live on campus, or access in-person services or facilities at SUNY Brockport, you are required to comply with NYS Immunization requirements for secondary education.
New York State requires students to provide immunization records by the first day of classes. We must receive your immunization records before classes start to avoid de-registration from classes.
Transfer Students: Immunization records are not automatically forwarded with your academic transcript. You must contact your previous college to request your immunization records or check their website for the ability to print off your immunization records to be sent to Brockport.
Required Immunizations
This vaccine requirement can be met through one of the following:
Two doses of MMR vaccine (given after your first birthday)
Two doses of Measles and one dose of Mumps and one dose of Rubella vaccine (given after your first birthday)
NYS Public Health Law 2167 became effective January 1, 2017. This law requires all colleges/universities to distribute information about meningococcal disease and vaccination. For more information regarding New York State PHL Section 2167 meningococcal disease and vaccinations please see meningococcal disease fact sheet. According to the NYS Public Health Law 2167 every student is required to address the meningitis requirement. To meet this requirement provide proof of vaccine given in last five years or decline the vaccine through My Hazen the patient portal
Proof of at least 1 dose of meningococcal ACWY vaccine (Menveo, Menactra or MenQuadfi)
OR a complete 2 or 3 - dose series of Meningitis B (Trumenba or Bexsero) vaccine
OR complete the waiver form refusing meningococcal vaccine which can be found on My Hazen under the “Forms” tab in the box labeled “Immunization Communication”. **
**If you are under the age of 26 and living in the residence halls, it is strongly recommended that you be vaccinated against meningitis and do not waive this requirement.
All new (including transfer) students at SUNY Brockport must complete a screening questionnaire for tuberculosis risk. The Tuberculosis Screening Form needs to be completed and submitted online through the MyHazen patient portal.
If you have previously tested positive for tuberculosis, you DO NOT need to have another tuberculosis test. You MUST upload the records for the treatment of your tuberculosis through MyHazen.
Go to the UPLOADS tab and select Immunization/TB Records.
If you received a secure message through MyHazen indicating that you need a tuberculosis test, please see your medical provider to have a tuberculosis test completed.
The Health Center Staff will provide you further information regarding testing in a secure message through MyHazen.
Once completed, access MyHazen to UPLOAD the laboratory report to Immunization/TB Records.
Tuberculosis Testing
If you require further testing, it should be completed prior to your arrival at SUNY Brockport.
Domestic Students
SUNY Brockport Health Center Staff will review the Tuberculosis Screening Forms. If further testing is required, we will reach out to the student through the MyHazen patient portal (secure messages).
Positive Tuberculosis Test Results
If you have previously tested positive for tuberculosis, you DO NOT need to have another tuberculosis test. You MUST upload the records for the treatment of your tuberculosis to MyHazen. Go to the UPLOADS tab and select Immunization/TB Records.
If you have never tested positive for tuberculosis previously, you must have a chest x-ray performed within the 12 months prior to enrollment. This is done to ensure that you are not able to spread active tuberculosis to others. This MUST be completed PRIOR to arrival at SUNY Brockport.

Tuberculosis (TB) Screening Form
All students must complete the “Tuberculosis Screening for International Students” form on your My Hazen patient portal under the Forms tab.
Students coming from countries with high level of Tuberculosis (per ACHA 2025 Guidelines) must complete the appropriate Tuberculosis screening, PRIOR to arrival to SUNY Brockport
IMPORTANT: Please complete these tests prior to arrival in the US to avoid extra expenses.
Within 30 days of arrival to the United States, obtain a TB Blood Test (also called interferon-gamma release assays or IGRAs) such as:
QuantiFERON-TB Gold Plus OR
T-SPOT TB Test (T-Spot)
PPD tests/Tuberculin Skin Test are not accepted.
TB Blood Test RESULT:
Must be an official lab test report.
Must be in English and include your name and date of birth (English format).
Must include the date of testing.
Must complete the “Tuberculosis Screening for International Students” form on your My Hazenpatient portal under Forms tab.
If your test is POSITIVE, you must get a chest x-ray as soon as possible and upload your results to yourMy Hazenpatient portal under selection “Immunization Records”.
If you have previously tested positive for TB, you must provide medical documentation along with a NEGATIVE chest x-ray. Upload your results to your My Hazen patient portal.
If you have already received a full course of treatment for TB, you must provide medical documentation confirming the start and end dates of treatment and the medications prescribed. Upload your documentation to your My Hazen patient portal under selection “Immunization Records”.
