Jonathan Hart is an accomplished healthcare leader with extensive experience in post-acute care, skilled nursing operations, and health system integration. Throughout his career, he has been dedicated to advancing quality of care, strengthening workforce development, and ensuring organizational sustainability in an increasingly complex healthcare landscape.
Since 2021, Jonathan has served as Vice President of Long-Term Care for Kaleida Health System, Western New York’s largest health system. In this role, he provides strategic oversight and operational leadership across multiple long-term care services. He is recognized for his ability to guide teams through regulatory change, implement quality improvement initiatives, and foster collaborative partnerships that benefit residents, staff, and the broader community.
Previously, Jonathan held leadership positions with Ascension Health, The McGuire Group, and several other nonprofit organizations. In these roles, he successfully led turnaround efforts, streamlined compliance processes, introduced innovative care delivery models, managed large-scale staff development initiatives, and launched programs designed to meet pressing community health needs. His leadership is distinguished by a balance of financial stewardship and a deep commitment to patient-centered care.
Jonathan earned a Master’s Degree in Public Administration from SUNY Brockport ’95 and a Bachelor’s Degree from St. John Fisher College ’91. He is also a licensed nursing home administrator in both New York and Pennsylvania and actively engages with professional associations dedicated to long-term care and senior health services.
In addition to his civilian career, Jonathan served in the United States Air Force from 2001 to 2011, as a member of the Medical Service Corps at the Niagara Falls Air Reserve Station in Niagara Falls, NY.
Beyond his professional responsibilities, Jonathan is a strong advocate for community health initiatives and has contributed to efforts that expand access to care for vulnerable populations.
